Appointments

We recommend that you contact us 2 weeks in advance to schedule an appointment unless you have a dental emergency.  If there is any change in your personal or dental insurance information, please inform us prior to your appointment date to avoid delays upon your arrival.  We ask that you arrive 10 minutes early to register (15 minutes for new patients who need to fill out additional forms).  Most appointments last for one hour unless specified. 

 

New Patients

For new patient initial visit, we provide diagnostic x-rays, comprehensive oral exam, periodontal screening for gum disease, TMJ (jaw) evaluation, photographs, and oral hygiene education.  Patient will also have the opportunity to address any dental concern with the doctor.  Please allow 90 minutes for your initial visit.

 

Emergencies

We have reserved daily time slots for dental emergencies, so please call us in the morning so we can accommodate you.

 

Canceling or Rescheduling an Appointment

If you need to cancel or reschedule an appointment, please notify us 48 business hours in advance so we can accommodate other patients who are on the waiting list.  There is a $50 charge per hour for appointment changes or cancellations made without 48-hour notice.  Patients who repeatedly miss their scheduled appointments might be dismissed from the practice.

 

Financial Policy

Full payment is due at the time of service unless prior arrangements have been made.  We give patients a goodwill written estimate prior to treatment to help them plan for the upcoming expenses.  Some treatment requires multiple procedures that must all be completed in a timely manner to avoid treatment failure.   

 

The guarantor is the person who takes responsibility for payment of all services rendered on his/her behalf or dependents regardless of any insurance involvement.  The guarantor, in most cases, is the patient or the parent or legal guardian of a minor child.  In the event of default, the guarantor promises to pay legal interest on the indebtedness, collection costs, and related legal fees.  The guarantor might not be the same as the insured (person who subscribed to the insurance).

 

Dental Insurance

We accept most PPO dental insurance plans.  There are two ways to submit claims to your insurance company.  Option 1 is to have patient pre-pay the full amount of all the dental services rendered.  The patient will file the claim to his/her insurance company for reimbursement.  The average time for claim processing is 60 days.

Option 2 is to sign a release form to assign dental benefits to the dentist.  After verification of insurance, patients will be given an estimate of their co-payment.  The guarantor pays the estimated patient co-payment directly to the dentist, and the dentist will file the claim with the insurance company.  If there is a discrepancy between the estimated and actual dental benefits received, the guarantor will either be billed for the balance or receive a refund for overpayment.

 

The insurance policy is an agreement between the insured or insured’s employer and the insurance company, not between the insurance company and this office.  If the insurance pays for a downgraded benefit, the guarantor will be responsible for the difference between the fee for the actual procedure and the downgraded fee.  If the insurance does not pay in full for any reason (employment changes, reduction in benefit, change in patient status, treatment costs exceeded annual maximum benefits), the guarantor will be responsible for the remaining balance of the treatment.  Therefore, it is the patients’ responsibility to understand their insurance benefit and policy prior to initiating treatment.  Most insurance plans do not cover cosmetic dentistry or implant surgery, therefore patients are required to pre-pay for these procedures unless there is written pre-approval from their insurance.

 

Patient co-payment is payable to the dentist at the time of service.   Most dental insurance plans will break down benefits according to 1) Preventive 2) Basic and 3) Major.  Each category will have a different coverage benefit based on percentage.  An annual deductible may be applied to your treatment before insurance benefit starts coverage.  Sometimes, insurance plans will downgrade a coverage based on least expensive alternative treatment (LEAT).  For example, resin or white filling will be downgraded to amalgam or silver filling.

Fees

Fees are subject to change with prior notice before treatment.  Balances remaining beyond 30 days from billing will have a late payment charge of $20 and accrue a monthly interest at a rate of 10% APR. There is a $35 processing fee for returned checks, and a $50 per hour fee for appointment changes, cancelations made without 48-hour notice.

 

Payment Terms

We accept cash, check, debit card, credit card – Visa, Mastercard, and Discover. 

We offer Care Credit financing.  If you prefer not to wait in line after your appointment, please ask us about our VIP Express checkout option.
